H & E) A B c Chapter 9 Rubella Retinopathy The differential diagnosis of a "salt and pepper" fundus includes rubella retinopathy. Because most children with rubella have a constellation of other signs, it is easy to determine the diagnosis. These patients have deafness and cataracts that are present at birth or develop postnatally. The lenses show persistent nucleated epithelial cells in the cortex. Other ocular signs are glaucoma (due to incomplete angle cleavage) and microphthalmos. Systemically, these children may have abnormalities of the brain and heart.
Giving a topical anesthetic before the dilating drops facilitates the dilation process. We find that telling the child it will hurt is far better, as they then later believe and trust you. Telling them that the drops do not hurt and then having them surprised by the stinging is far more devastating and sets a bad tone for the rest of the visit. When the patient's pupils have dilated, it is time to evaluate the fundus. Direct ophthalmoscopy is done initially to obtain an overall view. The optic nerve and macula can be examined to rule out anomalies.
